Responsibilities include advocating for patients and coordinating with families to ensure effective healthcare delivery. Strong candidates will hold a valid RN license along with a Bachelor's or Master's in Psychology, and experience in home healthcare. Formación

• Must possess RN state licensure in good standing.
• Bachelor or Masters degree in Psychology with experience in mental health is required.
• Experience with Medicare, home healthcare, and OASIS is essential.
• 1 year of Psych RN experience within the last 2 years is mandatory.

Responsabilidades

• Evaluate the patient and home health environment to determine required services.
• Serve as the patient's advocate by providing necessary information.
• Complete and synchronize OASIS visits and develop the plan of care.
• Involve the patient and family in developing the care plan.

Overview

Responsibilities

• Evaluates the patient & home health environment to determine required services
• Serves as the patient’s advocate by providing information
• Responsible for timely completion & synchronization of OASIS visits as required & developing the POC based on findings from evaluation visit
• Involves patient & patient’s family in developing the plan of care

Qualifications

We seek the following principal qualifications:

• RN state licensure in good standing
• Bachelor or Masters level degree in Psych & experience in mental health
• Medicare, home healthcare, and OASIS experience
• 1 year of Psych RN experience within the last 2 years
