Job summary

Hartford HealthCare at Home is seeking a Registered Nurse to provide skilled nursing care in clients' homes and community settings in CT. You will collaborate with the care team, maintain accurate documentation, and educate patients and families to promote health and prevent disease.

The role requires an Associate’s Degree or Nursing Diploma, CT RN licensure, and at least one year of nursing experience (home care or other).

Qualifications

• Education: Graduate of an Associate’s Degree or diploma program in nursing.
• Experience: One year nursing experience or one year nursing experience in a specialty.
• Licensure: Registered nurse with a license to practice in the state of CT.
• Preferred: Education:Graduate of NLN approved BSN program.
• Experience: Medical/surgery or home care experience.

Responsibilities

• Provide skilled nursing care to clients in home and community-based settings.
• Collaborate with care team to achieve exceptional outcomes.
• Maintain client records using standard documentation.
• Make follow-up visits to assess disease progress and educate patients/families.
• Coordinate care with providers and consult with physicians.
• Direct LPNs and Home Care Aides in carrying out plan of care.
• Refer client/family to community resources as needed.
• Attend in-service education programs.
