Job summary

ALC Health Care Services is seeking a PRN Physical Therapist Assistant to deliver home health PT services on an as-needed basis across the Illinois service area. You will follow physician plans, assess patients at home, and adapt treatments to mobility and functional goals.

This role emphasizes scheduling flexibility, direct communication with the Home Health Director, and ongoing progress reporting. A PTA license and CPR certification are required, with experience in home health preferred.

Qualifications

• Graduate of PTA program approved by the American Physical Therapy Association.
• Current state license or certification as required by law.
• Two years of PTA experience; home health preferred.
• Demonstrates good verbal and written communication and organization skills.
• Maintains CPR certification.
• Must have a valid driver’s license and insured vehicle.
• Possesses general liability/workers’ comp coverage/certification.

Responsibilities

• Provide PT services per physician's plan of care, assess functional status and environment.
• Evaluate strength, mobility, ROM and rehab potential; direct therapy sessions.
• Instruct patients and caregivers in use of therapeutic appliances.
• Identify needs for other home health services and refer as needed.
• Prepare and submit clinical progress summaries and discharge plans.
• Offer PT consultation to patients' families when indicated.
• Provide in-service education to staff as required.
• Supervise PT assistants per policy and state regulations.
• Instruct and evaluate home health aides every two weeks when therapy is sole skilled service.

This is a PRN position with the following pay rates:

• Standard visits $55.00

The Physical Therapist Assistant employed by ALC Home Health is responsible for completing physical therapy services in adherence to all conditions outlined in the Personnel Service Agreement.

The PRN Physical therapist Assistant position offers:

• Flexibility and Convenience to control your own schedule and geographic area of your patients.

• Administrative support to schedule and track visits utilizing Emergency Medical Response technology with direct communication to Home Health Director.

Essential Job Functions/Responsibilities

1. Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:

• Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

• Assesses for muscle strength, mobility, gait, ROM—potential for rehab.

• Directing physical therapy treatment.

• Instructing patients and families/caregivers in the use and care of therapeutic appliances.

• Determining priority needs for physical therapy.

• Reporting to physician patient’s reaction to treatment or changes in condition.

2. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.

3. May train patient in the use of prosthetic device.

4. Identifies patient and family/caregiver needs for other home health services and refers as necessary.

5. Prepares and submits clinical and progress summaries based on the attainment of goals.

6. Participates in discharge planning for patient.

7. Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.

8. Provides physical therapy consultation to home families/caregivers when indicated.

9. Provides in-service education programs for nursing organization personnel as needed.

10. Participates in peer consultation process.

11. Supervises physical therapy assistants according to organization policy and state regulations.

12. When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

Position Qualifications

1. Graduate of an accredited physical therapy assistant program with either an Associate's degree in science or applied science that is approved by American Physical Therapy Association, Inc.

2. Current certification or license as required by state law.

3. Two years of appropriate experience as a physical therapy assistant. Community/home health experience is preferred.

4. Demonstrates good verbal and written communication and organization skills.

5. Possesses and maintains current CPR certification.

6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

7. Must possess Independent general liability workman’s comp coverage/cert.
