Job summary

BAYADA Home Health Care is seeking a Home Health PTA to join our Atlantic County Visits office in Egg Harbor Township, New Jersey. The PTA will provide in-home therapy services to adult and geriatric clients on a per-visit basis, traveling across designated territories within Atlantic County.

We offer full-time, part-time, and per-diem schedules. A minimum of one year of recent PTA clinical experience is required, with a strong emphasis on compassionate, reliable care and 1:1 patient interaction

Qualifications

• Current RN license in New Jersey.
• Minimum of 1 year recent clinical experience.
• Home health experience preferred but not required.
• Graduation from an approved nursing program.
• Ability to travel to cases as assigned.

Responsibilities

• Follow a designated care plan per patient needs.
• Make home visits to clients in designated territories.
• Perform duties including medication administration, wound care, treatments, and procedures.
• Monitor client conditions and report changes to supervisor.
• Follow up with, execute, and document doctors' orders.

BAYADA Home Health is looking for compassionate and dedicated Physical Therapist Assistant (PTA),to join our team in our Atlantic County Visits office. This office services our adult and geriatric clients on a per visit basis in territories throughout Atlantic county.

BAYADA offers full-time, part-time and per-diem schedules.

One year prior clinical experience as a licensed PTA is required.

Physical Therapist Assistant (PTA) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Physical Therapist Assistant (PTA):

• A current license as a Registered Nurse in New Jersey.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:
$40.00 - $47.00 per point
