Job summary

Elara Caring is seeking a Home Health Physical Therapy Assistant to provide one-on-one PTA services in patients’ homes across Saline, Ann Arbor, and Dexter areas in Michigan.

You will deliver high-quality therapy under a supervising PT, support Plan of Care, and help patients regain mobility and independence. This full-time role offers a stable schedule, travel within the service area (up to 50%), competitive compensation, and tuition reimbursement.

Qualifications

• Graduate of an accredited PTA program.
• Current, unrestricted PTA license in the state of practice.
• At least 6 months PTA experience preferred.
• Reliable transportation, valid driver’s license, and auto insurance.
• Willingness to travel within the service area (up to 50%).
• Ability to lift 50–100 lbs and perform physical tasks related to patient care.

Responsibilities

• Deliver high-quality physical therapy services under the direction and supervision of a Physical Therapist.
• Support the patient’s Plan of Care and report changes in condition to the supervising PT.
• Ensure care aligns with state practice guidelines and physician orders.
• Help patients achieve measurable progress toward their mobility and independence goals.
• Teach patients and caregivers how to safely use assistive devices.

What You’ll Do

As a valued member of our therapy team, you will:

• Deliver high-quality physical therapy services under the direction and supervision of a Physical Therapist
• Support the patient’s Plan of Care and report changes in condition to the supervising PT
• Ensure care aligns with state practice guidelines and physician orders
• Help patients achieve measurable progress toward their mobility and independence goals
• Teach patients and caregivers how to safely use assistive devices
• Provide education on therapy-related home exercise programs
• Promote Elara Caring’s mission, values, and commitment to excellence
• Work in a physical, hands‑on environment that requires lifting and mobility
• Work occasional irregular or on‑call hours as needed

What We’re Looking For

• Graduate of an accredited Physical Therapist Assistant program
• Current, unrestricted PTA license in the state of practice
