Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapy Assistant for a role based in Melrose Park, Illinois. The position requires a license as a Physical Therapist Assistant, an associate degree, and at least one year of clinical experience.

Clinicians will collaborate within Residential Home Health and Hospice for patient care plans, delivering hands-on therapy, driving to patient locations, and meeting charting deadlines.

Qualifications

• Associate degree in physical therapy from an accredited college or technical school is required.
• Minimum one year of experience in an acute care and/or rehabilitative setting preferred; previous home health experience preferred.
• Must hold, in good standing, a license/registration as a Physical Therapist Assistant, issued by the State in which you work.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to consistently meet critical deadlines for charting; self-manage time and schedule.
• Experience with tablets, mobile devices and EMR software.
