Job summary

Angels Care Home Health is hiring a Physical Therapist to assess and treat home health patients in the Winfield area. You will plan, implement, and evaluate therapy programs to restore or maintain patient well-being, while coordinating with an interdisciplinary team.

The role offers a Monday–Friday schedule, full benefits (health, dental, vision), a vehicle allowance, and performance bonuses for full-time staff. Experience as a PT and current state licensure are required.

Qualifications

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Plans, implements, and evaluates patient care plans to restore or maintain patient well‑being.
• Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation.
• Collaborates with interdisciplinary team.
