Job summary

Elara Caring is hiring a Physical Therapist to join our home-based care team. You will deliver direct patient care, assess functional needs, and design individualized plans of care in patients’ homes across the coverage area.

The role requires a PT license, 1+ year in healthcare, and travel up to 50%. We offer a sign-on bonus of $7,500, tuition reimbursement for full-time staff, and opportunities for advancement within a collaborative, supportive environment.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.
• Designs and implements a plan of care for patients based on a thorough assessment.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Documents evaluations, treatment goals, and plans; updates documentation and maintains signatures.
• Provides direction and leadership to staff to promote physical therapy in the community.
• Operate in a physically demanding, high-stress environment.
• Handle, lift and transfer patients as part of full range of body motion.
• May include irregular hours including call hours if applicable.
