Job summary

Elara Caring is hiring a Physical Therapist to provide in-home care in Hillsdale County, Michigan. You will assess patients, develop personalized treatment plans, and deliver high-quality PT services in the home environment.

You’ll collaborate with physicians and care teams, document meticulously, and supervise therapy staff as needed. APTA-approved program, active PT license, and a minimum of one year of healthcare facility experience are required.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
