Job summary

Commonwealth Home Health, part of LHC Group and Optum, seeks a Physical Therapist to evaluate patients in their homes and develop care plans focused on mobility, endurance, pain management, and function. You will implement interventions, monitor progress, and collaborate with nurses and physicians to optimize outcomes.

A $5,000 sign-on bonus is available for full-time external hires; state licensure and a valid driver's license are required, with the ability to travel to patient homes as needed.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to dependable vehicle or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs and assists in the development of the plan of care.
• Provides PT services within scope of practice and plan of care, coordinating with the health care team.
• Observes and reports patient responses and changes to supervising nurse/physician.
• Treats patients to relieve pain and improve or restore function while maintaining maximum functioning.
• Instructs patients and family on plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment and documents progress.
