Qualifications

• Active, unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a healthcare team.

Responsibilities

• Plan and administer prescribed PT at patients' homes to improve function.
• Assess and document patients’ physical strengths to guide goals.
• Develop and implement rehab plans per physician's Treatment Plan.
• Provide clear care instructions to PTAs and Home Health Aides; oversee documentation.
• Document observations, interventions, and evaluations promptly.
• Report progress to physician, Clinical Manager, care team, patient, family; submit required records.
