Job summary

CenterWell Home Health in Pullman, WA is seeking a Home Health Physical Therapist to improve patients’ strength, mobility and independence through in-home visits. You will assess, document, and implement rehabilitation plans with a multidisciplinary team to optimize outcomes for patients and families.

Responsibilities include planning PT treatments, guiding care teams, and ensuring timely documentation and progress reporting.

Qualifications

• Current and unrestricted Physical Therapy license required.
• Current CPR certification required.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to attain highest physical function.
• Assess and document patients' strengths to support physician evaluation and guide goals.
• Develop rehabilitation program aligned with physician plan and patient goals.
• Provide PTAs/Home Health Aides with clear care instructions and supervise documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report progress to physician, Clinical Manager, care team, patient and family; submit required reports.
