Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in their own homes. You will assess, plan and deliver personalized PT care, working with a multidisciplinary team to meet rehab goals.

You will document progress, communicate with physicians and family, and participate in a driver safety program as applicable. The role is full-time, 40 hours per week, with pay range $102,300 - $140,800 per year.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to restore function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehab program aligned with physician's plan of treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
