Job summary

BAYADA Home Health Care is seeking a Physical Therapist (PT) for full-time, part-time, or per diem home health visits in Southern Chester County. New graduates welcome. You will join a multidisciplinary team to provide rehabilitative care and support for home-based clients.

The PT will perform assessments, develop care plans, educate families, and document care using electronic records. Benefits include comprehensive plans and ongoing training.

Qualifications

Valid Pennsylvania Physical Therapist license.
Graduation from an APTA/CAAHEA-approved PT program.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational and problem-solving skills.
Solid computer skills; EMR experience preferred.

Responsibilities

Make home visits to clients in designated territories.
Perform diagnostic tests and mobility/strength assessments.
Implement wound care and evidence-based dressings as needed.
Develop and revise individualized care plans including PT interventions.
Educate clients and families on rehabilitation and safety at home.
Document observations and interventions using a tablet/digital system.
