Advanced Nursing Concepts is seeking a licensed physical therapist to provide one-on-one home health care across our service area. You will assess patients, develop and carry out plans of care, and collaborate with physicians to optimize outcomes.

Travel to patient homes is required; you must have a valid driver's license, reliable transportation, and CPR certification. We offer flexible scheduling, a 401(k) plan, and additional supplemental benefits to support clinicians.

Qualifikationen

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Licensed to practice in Florida.
• At least two years of physical therapy experience; home health preferred.
• Excellent verbal and written communication and organizational skills.
• Current CPR certification.
• Must hold a valid driver's license and have reliable transportation; able to drive in varying conditions.

• Assess patients' functional status, strength, mobility, gait, ROM, and rehab potential in home settings.
• Carry out the physician's plan of care for physical therapy.
• Instruct patients and families on therapeutic devices.
• Document care accurately and on time, including progress summaries.
• Coordinate with physicians and report responses to treatment or changes.
• Identify needs for other home health services and refer as appropriate.
• Participate in discharge planning and case conferences.
• Provide consultation to families and staff training.
• Supervise physical therapy assistants per policy and state regulations.
• Monitor home health aide care every two weeks when therapy is the only skilled service.
• Take on other duties as assigned.

• Schedule: PRN, successful candidates will have 2-3 days of availability weekly from 7:00a-4:00p
• Employment type: W-2 or 1099
• Mileage: Currently $0.60 per mile (W-2)
• Reports to: Branch Director

What You'll Do

• Assess each patient's functional status, muscle strength, mobility, gait, ROM, and rehab potential, including the home environment
• Carry out the physical therapy plan of care set by the physician
• Instruct patients and families on therapeutic appliances and prosthetic devices
• Document care accurately and on time, including clinical and progress summaries
• Coordinate with physicians and report patient response to treatment or changes in condition
• Identify needs for other home health services and refer as appropriate
• Participate in discharge planning and case conferences
• Provide consultation to families/caregivers and inservice education to organization personnel
• Supervise physical therapy assistants per organization policy and state regulations
• Check in on home health aide care every two weeks when therapy is the only skilled service
• Take on other duties as assigned

Qualifications

• Possesses a degree from a baccalaureate or Master's program in physical therapy approved by an accredited organization.
• Licensed to practice as physical therapist within the state, if applicable.
• Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.
• This position requires regular travel by personal vehicle between patient homes, the office, and other locations throughout the assigned service area. The clinician must hold a valid driver's license, maintain reliable transportation and current auto insurance, and be able to drive safely for extended periods in varying weather and traffic conditions.

Physical Demands

This role requires the following, with or without reasonable accommodation:

• Prolonged walking and standing
• Lifting and carrying up to 25 lbs frequently and up to 50 lbs occasionally
• Lifting, positioning, and transferring patients (with assistance for patients over 50 lbs, per agency safe-handling policy)
• Reaching, stooping, bending, kneeling, and crouching
• Visual acuity and hearing sufficient to perform required duties

Health Requirements

Required before providing patient care:

• Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease
• Documented negative TB test (TST or TB blood test) taken within the past 12 months
• TB risk assessment and symptom evaluation upon hire
