Qualifications

• Must hold a valid California Physical Therapist license.
• CPR or BLS certification required.
• Driver’s license and auto insurance.
• Physical and TB test within the last year.
• Professional liability and business license (if applicable).

Responsibilities

• Provide skilled PT care: conduct in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement therapeutic interventions: focus on functional strengthening, gait training, balance and fall risk reduction.
