Job summary

Concierge Home Care seeks a Home Health Physical Therapist (PT) to deliver one-on-one care in patients' homes across Bradenton area. Flexible schedule with autonomy and income potential up to $120k+ annually.

You will evaluate patients, design individualized plans, and collaborate with physicians while leveraging AI-assisted charting and WellSky/Kinnser EMR to maximize patient time with minimal documentation.

Qualifications

• Active, unencumbered Florida PT license.
• Valid driver's license and reliable transportation.
• Home Health PT – Flexible Schedule.

Responsibilities

• Evaluate patients and develop individualized home-based plans.
• Provide therapeutic exercises and mobility training at patients’ homes.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise PTAs and complete streamlined documentation.
• Health insurance starts after 30 days, PTO accrues day one.
• Flexible employment options: full-time, PRN, or 1099.
