Halcyon Home LLC in Fort Worth, Texas, is seeking a Home Health Field Physical Therapist PRN to support our geriatric patients in Tarrant County and the surrounding area. You will perform patient assessments, develop care plans, and collaborate with a care team to deliver compassionate, high-quality home health services.

The role emphasizes strong clinical skills, excellent communication, and the ability to work independently in a fast-paced, home-based setting.

Qualifications

• Current PT license in good standing.
• Experience in home health or related clinical setting preferred.
• Strong clinical assessment and documentation skills.
• Excellent interpersonal and communication abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Performs ongoing clinical assessments (e.g., OASIS) to identify patient needs and determine home care suitability.
• Develops and evaluates the plan of care with the patient and caregiver(s).
• Documents and updates clinical notes, orders, and records in a timely manner; revises POC as needed.
• Provides physician-ordered services and oversees team members as appropriate.
• Applies infection control and standard precautions in patient care activities.
• Supervises and guides PT assistants and home health aides when applicable.
• Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders per medical referral.
• Communicates with physicians and health practitioners involved in the plan of care.
• Meets continuing education requirements of the agency and licensing board.
• Instructs patients and families on transfers and equipment use; provides education.

Key Responsibilities:

A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of Homecare Homebase software is a plus!

• Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
• Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
• Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, in the performance of patient care that is consistent with patients’ needs.
• Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
• Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
• Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
• Meets mandatory continuing education requirements of the Agency and licensing board.
• Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
• Provides patient, caregiver, and family counseling; Provides patient and caregiver education.

Requirements

• Current PT license
• Knowledge or experience utilizing Homecare Homebase EMR would be a plus
• Experience in home health or related healthcare setting preferred
• Strong clinical and assessment skills
