Job summary

PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our home health team in Texas. You will evaluate patients in their homes, develop and implement individualized treatment plans, and guide families in safe exercises to restore mobility and reduce pain.

The ideal candidate holds an active Texas license, a graduate degree in Physical Therapy, and experience in home health. You will collaborate with an interdisciplinary team, document progress, and provide patient-centered care

Qualifications

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage for Texas PT license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Excellent communication and adaptability in patient care.
• Commitment to evidence-based treatment techniques.

Responsibilities

• Evaluate patients’ physical abilities in a home health setting.
• Develop and implement individualized treatment plans.
• Provide therapeutic interventions including exercise and gait training.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans for optimal outcomes.

Essential Job Functions:

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists
• Conduct home safety assessments and make recommendations to prevent falls or re-injury
• Deliver compassionate care while empowering patients to achieve greater independence
