PARS Therapy in Red Oak, TX is seeking a compassionate Physical Therapist (PT) to join our dedicated home health team. You will assess patients in their homes, design personalized treatment plans, and help them regain mobility and independence.

This full-time role requires Texas licensure, liability insurance, and a graduate degree in Physical Therapy. Prior home health experience is preferred, with strong communication and evidence-based practice.

Formación

• Active Texas Physical Therapist license or eligible
• Professional liability insurance coverage in TX
• Graduate degree or higher in Physical Therapy
• Experience in home health or rehabilitative settings preferred
• Strong ability to adapt care approaches to patient needs
• Up-to-date knowledge of evidence-based treatment techniques and best practices
• Excellent communication skills with patients and families

Responsabilidades

• Evaluate patients in a home health setting and assess functional limitations
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs and safety
• Monitor progress and adjust plans to achieve outcomes
• Document evaluations, sessions, and progress using digital platforms
• Collaborate with physicians, nurses, and other therapists
• Perform home safety assessments and recommend fall-prevention measures
• Deliver compassionate, patient-centered care to empower independence

Essential Job Functions:

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists
• Conduct home safety assessments and make recommendations to prevent falls or re-injury
• Deliver compassionate care while empowering patients to achieve greater independence

Requirements:
