PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our dedicated home health team in Navasota, TX. The PT will evaluate patients in their homes, develop individualized treatment plans, and implement interventions to improve mobility, reduce pain, and restore function.

The role emphasizes patient education, safety, and collaboration with an interdisciplinary team to deliver evidence-based care and empower patients toward greater independence.

Qualifications

• Active Physical Therapist license in Texas
• Professional liability insurance coverage for Texas PT license
• Graduate degree or higher in Physical Therapy
• Experience in home health or rehabilitative settings preferred
• Excellent communication and patient education skills

Responsabilités

• Evaluate patients in home health settings and determine functional goals
• Develop and implement individualized PT treatment plans

PARS Therapy is seeking a compassionate and skilled Physical Therapist (PT) to join our dedicated home health team in Navasota, TX. Essential Job Functions:

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists
• Conduct home safety assessments and make recommendations to prevent falls or re-injury
• Deliver compassionate care while empowering patients to achieve greater independence

Requirements:

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage specific to your TexasPhysical Therapy license
