Job summary

PARS Therapy in Humble, TX is seeking a compassionate Physical Therapist to join our home health team serving Kingwood, Humble and surrounding communities. You will evaluate patients at their homes, design and implement individualized treatment plans, and provide interventions such as therapeutic exercise, gait training, manual therapy, and balance retraining.

You will educate patients and families, document progress, and collaborate with physicians and other professionals.

Qualifications

• Must have active Texas Physical Therapist license.
• Professional liability insurance coverage for TX.
• Graduate degree in Physical Therapy from accredited program.
• Experience in home health or rehabilitative settings preferred.
• Strong communication and patient education skills.

Responsibilities

• Evaluate patients’ physical abilities in a home health setting.
• Develop and implement individualized treatment plans.
• Provide therapeutic interventions: exercise, gait training, manual therapy, balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans for optimal outcomes.

Essential Job Functions:

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists
• Conduct home safety assessments and make recommendations to prevent falls or re-injury
• Deliver compassionate care while empowering patients to achieve greater independence

Requirements:
