Job summary

Elara Caring in Quincy seeks a Physical Therapist for one-on-one home visits. You’ll assess, plan, and deliver therapy directly in patients’ homes, traveling within the territory and coordinating with a clinical team.

The role offers per diem scheduling, supports ongoing education, competitive compensation, and benefits; travel-required, great autonomy, and opportunities for advancement within a mission-driven home care company.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Reviews patients’ functional needs and adjusts care plans to ensure optimal outcomes.
• Designs and implements a plan of care based on thorough assessment.
• Documents evaluations, treatment goals, and plans; maintains prescriptions and signatures.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Promotes the company’s philosophy and policies to ensure quality of care.

• Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care
• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.
• Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
• Conducts therapy practice within defined standards of care.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
• Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community
• Physically demanding, high stress environment
• Full range of body motion including handling, lifting and transfer of patients
• Potentially work irregular hours including call hours if applicable

The base salary for this position is $60.00 to $70.00 per point, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity. Elara Caring determines compensation based on education, job related knowledge, skills, training, and experience. Our compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location.You will report to the Branch Director, Clinical Manager, Clinical Supervisor or designee.
