Qualifications

• Active Physical Therapist license in TX (Texas).
• Professional liability insurance coverage for TX license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to a variety of patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.
• Excellent communication skills and professional demeanor.

Responsibilities

• Evaluate patients' physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Accurately document evaluations, treatment sessions, and progress notes in digital systems.
• Collaborate with physicians, nurses, and other therapists.
