Job summary

Pride Health is seeking a Physical Therapist (Home Health) to support our client's healthcare facility in Bourne, MA. The role offers a Mon-Fri 8:00 AM-4:30 PM schedule with a 13+ month contract in a local setting, traveling as needed within the Cape Cod area.

A valid PT license and at least 1 year of home health experience are required, with HomeCare HomeBase (HCHB) experience a plus. The clinician will evaluate, treat, and document patient progress using EMR, coordinate with the care team, and

Qualifications

• Active Physical Therapist license.
• At least 1 year of home health experience; experience with HomeCare HomeBase (HCHB) is a plus.

Responsibilities

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.
• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.
• Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems.
• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.
• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.
• Participate in case conferences, care coordination activities, and team collaboration.
• Ensure compliance with CMS regulations, infection control standards, and agency policies.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.
