Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled evaluation and treatment in home health settings across Clinton, IL and surrounding areas. The role emphasizes helping patients regain mobility and independence through targeted therapies.

Responsibilities include implementing specialized programs, collaborating with a care team, and documenting progress per physician orders. The position offers competitive compensation and comprehensive benefits.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities to improve mobility and self-care.
• Implement Specialty Programs including Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement, and Total Shoulder Replacement.
• Administer skilled care to clients per physician orders and established plan of care under supervision of the Branch Director.
