Job summary

Aspire Home Healthcare Inc is seeking a licensed Physical Therapist (PT) to join our home health team in the San Diego area. The role involves evaluating and treating patients in their homes with one-on-one care, developing treatment plans, and coordinating with the clinical team.

We offer flexible scheduling, per-visit pay starting at $85, mileage reimbursement, and growth opportunities across multiple states. A patient-first environment and supportive leadership are emphasized.

Qualifications

• Active PT license in CA is required.
• Minimum 1 year PT experience preferred.
• Physical exam within the last 6 months.
• Current TB test (within 12 months) and CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures.
• Evaluate patient condition and establish measurable treatment goals.
• Develop and implement individualized treatment plans.
• Supervise and coordinate care with PTAs and HHAs.
• Document patient progress, updates, and treatment notes accurately and on time.
• Educate patients and families on exercises, mobility, and use of adaptive equipment.
• Evaluate home environments and recommend modifications to improve safety and independence.
• Ensure all services meet HIPAA standards for confidentiality.
• Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care.
