You will assess, design, and implement evidence-based treatment plans, collaborating with a multidisciplinary team to ensure optimal recovery and meaningful outcomes for clients in home health

Qualifications

• Active Physical Therapist license in Michigan.
• Experience in home health is a plus but not required.
• Strong communication and patient-family interaction skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation for home visits.
