Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress.

• Evaluates the patient's functional status and therapy needs, assisting the physician in developing the plan of care.
• Observes and reports the patient's response to treatment and any condition changes to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning according to standards of practice.
• Instructs and informs healthcare team members, patients, and families about the plan of care, limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in IL.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle.
