Job summary

ChristianaCare HomeHealth is seeking a dedicated Physical Therapist to deliver skilled services in patients' homes, helping them regain mobility and independence while supporting meaningful health goals. You will develop individualized treatment plans, educate patients and caregivers, and collaborate with physicians and nurses to achieve optimal outcomes.

A minimum of 1 year of PT experience, current licensure, a valid driver's license, and home health experience are preferred.

Qualifications

• Minimum 1 year of Physical Therapist experience.
• Current and valid Physical Therapist licensure.
• Home health experience preferred.
• Valid driver's license and active automobile insurance required.
• Strong clinical, communication, and patient education skills.

Responsibilities

• Deliver skilled physical therapy services to patients in their homes.
• Develop and implement individualized treatment plans.
• Educate patients and caregivers on exercises, safety measures, and mobility strategies.
• Collaborate with physicians, nurses, and other healthcare professionals to achieve optimal patient outcomes.
• Document care accurately and in accordance with regulatory standards.
