Enhabit Home Health & Hospice is seeking a Physical Therapist to evaluate and treat patients at home, enhancing mobility and independence under physician orders. The role includes teaching families and coordinating with the care team to implement the plan of care.

This position emphasizes comprehensive in-home therapy, adherence to safety protocols, and collaboration with the Branch Director to ensure high-quality patient outcomes in a supportive, growth-oriented environment.

Formación

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsabilidades

• Examine and treat patients with physical impairments using physical modalities, to improve mobility and independence.
• Assist patients with daily living and self-care goals per care plan.
• Administer skilled care per physician orders under Branch Director supervision.
• Educate families and health-care team members as part of the care plan.
