Job summary

Beth Israel Lahey Health's Home Health team in Massachusetts seeks a Physical Therapist to plan, coordinate and deliver in-home care. You will collaborate with patients, families and the care team to meet individual goals in the home setting.

Responsibilities include initial assessments, developing a patient-specific plan of care, initiating and revising treatments, and ensuring OASIS and HIPAA compliance. MA PT license and BLS certification required; DPT preferred.

Qualifications

• Degree from an accredited Physical Therapy program; DPT preferred.
• Massachusetts Physical Therapist license current and active.
• BLS certification current.
• 2+ years PT experience, including home health/public health.
• OASIS experience preferred.

Responsibilities

• Coordinate care for assigned patient caseload, including supervising PTA staff (if applicable) and managing patient condition changes.
• Complete initial patient and family assessment and develop a patient-specific plan of care.
• Initiate and revise the plan of care as patient needs change; administer therapies per physician orders.
• Communicate with the physician about patient needs and changes; obtain orders as required.
• Maintain HIPAA confidentiality and complete timely OASIS documentation.
