Role Description

The Home Health Physical Therapist will provide in-home physical therapy services to patients in the Ft. Worth, TX area, delivering individualized treatment plans that support mobility, safety, and functional independence. Day-to-day responsibilities include conducting patient assessments, developing and adjusting rehabilitation plans, providing therapeutic exercises and interventions, and educating patients and caregivers on home-based care strategies and equipment use. The role involves close collaboration with physicians, nurses, and other interdisciplinary team members to coordinate care and document progress in accordance with regulatory and agency standards. This is a full-time hybrid role, based in Dallas, TX, with a combination of in-home patient visits and some work-from-home for documentation, care coordination, and virtual team meetings.

Qualifications

• Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and implement evidence-based treatment plans.
• Experience in Patient Care and Home Care, including delivering services in patient homes and promoting safety and independence.
• Relevant knowledge of Medicine as it relates to home health and post-acute care, including common diagnoses and precautions.
• Current state licensure as a Physical Therapist and graduation from an accredited Physical Therapy program.
• Prior experience in home health or community-based care preferred; hospice or palliative care exposure is a plus.
• Strong communication, documentation, and time-management skills, with the ability to work autonomously in the field.
• Valid driver’s license, reliable transportation, and ability to travel within the Dallas/Benbrook service area.
