Job summary

HealthPro Heritage in Vernon, CT is seeking a dedicated Home Health Physical Therapist Assistant (PTA) to join our team and support patients in Vernon and surrounding areas.

You will implement treatment plans, document progress, and collaborate with Physical Therapists and care teams. Flexible PRN schedules and competitive per-visit rates help you build a schedule that fits your life while delivering quality care in diverse settings.

Qualifications

• Associate degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
• Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems.

Responsibilities

• Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Monitor and document patient progress; educate patients and families on treatment protocols.
• Accurately document patient treatments, progress, and responses in line with policies and standards.
• Collaborate with Physical Therapists and other healthcare professionals to ensure coordinated care.

Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.
