Job summary

HealthPro Heritage is seeking a Home Health Physical Therapist Assistant for a full-time role in Mystic, CT and surrounding areas. A $2,500 sign-on bonus is offered, with high per-visit rates and a flexible schedule within a home health setting serving adult patients.

The position requires an Associate’s degree in Physical Therapist Assisting, state licensure or eligibility, and strong clinical skills. Benefits include medical, dental, vision, life insurance, 401K, and CEU access.

Qualifications

• Associate’s degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Strong assessment, diagnostic and therapeutic abilities with excellent communication.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists. Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
• Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
• Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
