Job summary

CenterWell Home Health is seeking a licensed Physical Therapist to provide in‑home therapy to patients in the Stone Mountain area. You will deliver individualized treatment plans, promote functional improvement, and help patients regain independence in the comfort of their homes.

You will document care, collaborate with a clinical team, and participate in ongoing education to maintain high standards of patient outcomes.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric population changes.
• Read, write, and speak fluent English.

• Administer non‑complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.

• Attend educational meetings and coordinate department in‑services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.

Compensation

• Pay Per Visit/Unit Rate: $41.00 - $57.00
• Scheduled Weekly Hours: 1
• Pay Range: $64,000 - $87,500 per year (full time)

• TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be screened for TB.
• Driving Statement: This role is part of Humana’s driver safety program and requires an individual to have a valid state driver's license and to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
