CenterWell Home Health seeks a Home Health Physical Therapist Assistant to help patients regain strength and independence in their homes under the supervision of a Physical Therapist. You will perform prescribed treatments, document care, and participate in team meetings to coordinate patient progress.

A valid PT license, driver's license, CPR certification, and knowledge of geriatric care are required; one year of experience is preferred.

• Comprehensive knowledge of geriatric physical, emotional, social changes.
• Read, write, and speak fluent English.

Responsabilidades

• Administer PT treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapy, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.

As a Home Health Physical Therapist Assistant, You Will

Required Qualifications

Pay Per Visit/Unit Rate

$41.00 - $57.00

TB Statement

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
