Physical Therapist Assistant – Home Health

We’re seeking a compassionate, dedicated Physical Therapist Assistant to provide exceptional home‑health care in patients’ homes.

Key Responsibilities
  • Provide nursing care, including medication administration, wound care, and treatments.
  • Monitor patients’ conditions, report changes, and assist in developing personalized care plans.
  • Educate patients and families on health management, medications, and self‑care techniques.
  • Collaborate with the healthcare team to ensure highest quality care for each patient.
Primary Job Duties
  • Administers prescribed physical therapy treatments to patients at all life stages.
  • Assists with program development and discipline‑specific operations.
  • Communicates and documents information in accordance with agency policies and discipline guidelines.
  • Contributes to program effectiveness.
  • Organizes and performs work effectively and efficiently.
  • Maintains and adjusts schedule to enhance agency performance.
  • Demonstrates daily commitment to the values of the agency.
  • Demonstrates positive interpersonal relations with all agency members.
  • Maintains and promotes customer satisfaction.
  • Effectively demonstrates the mission, vision, and values of the agency.
Job Specifications
  • Education: Associate Degree from an accredited physical therapy assistant program.
  • Licensure / Certifications: Licensure in the state (NC/SC) and CPR certification.
  • Experience: Minimum 6 months.
  • Essential Technical / Motor Skills: Knowledge of PT procedures, equipment use, patient transfer, manual therapy techniques.
  • Interpersonal Skills: Ability to interact professionally with patients, families, and physicians.
  • Essential Physical Requirements: Standing/walking, lifting >50 lbs, transferring >100 lbs, repeated reaching, bending, stooping; occasional sitting and fine motor tasks.
  • Essential Mental Abilities: Apply PT principles, develop treatment programs, prioritize duties, learn new skills, use technology.
  • Essential Sensory Requirements: Adequate vision and clear communication.
  • Exposure to Hazards: Noise, congestion, communicable disease, blood‑borne pathogens, patient assault, improper body mechanics, hazards.
  • Hours of Work: 40‑hour work week, variable Monday‑Friday schedule, weekends and holidays as needed.
  • Population Served: Adolescents, adults, geriatrics.
  • Transportation: Valid North Carolina driver’s license and operational vehicle.
