Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to administer PT plans in patients' homes under the supervision of a Physical Therapist. You will perform therapeutic exercises and modalities, document visits, and participate in team in-services while ensuring safety and infection control standards.

The role requires a current Physical Therapy license (PTA/PT), CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• Current state license to practice Physical Therapy (PTA or PT)
• Valid driver’s license, auto insurance and reliable transportation
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Comprehensive knowledge of physical, emotional, social, and biological changes in geriatric patients
• Preferred: One year of PTA/home health experience

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in patients' homes
• Deliver non-complex manual therapy exercises, therapeutic massage, and modalities as prescribed
• Document client records and submit clinical documentation weekly
• Attend educational meetings and coordinate department in-services
• Notify supervisor of equipment failures, supply shortages, or changes in staff or patients
• Apply infection control and universal precautions in patient care
• Use skills to make a measurable impact on patient independence

As a Home Health Physical Therapist Assistant, you will:

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.
• Use your skills to make an impact.

Required Qualifications

• Current state license to practice Physical Therapy
• Valid driver's license, auto insurance and reliable transportation
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population

Preferred Qualifications

• One year of experience as a Physical Therapy Assistant
• Home Health experience

TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

Pay Range

Scheduled Weekly Hours 40

Pay Range. The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $76,800 - $105,000 per year
