CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver patient-centered therapy in patients' homes, under the supervision of a Physical Therapist. The role emphasizes functional improvement and coordination with a broader care team.

Requirements include an active Physical Therapy license, valid driver's license with reliable transportation, CPR certification, and strong geriatric care knowledge.

Qualifications

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Administer non-complex manual therapy, therapeutic exercises, and modalities.
• Document client records and submit clinical documentation on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff or client changes.
• Apply infection control and universal precautions in patient care.

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. As a Home Health Physical Therapist Assistant, you will:

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.

Pay Range

• - $47.00- $65.00 pay per visit/unit
• - $69,100 - $94,500 per year base pay

Additional Information

TB Statement:

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement:

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
