Job summary

BAYADA Home Health Care is seeking a Physical Therapist for a full-time home health role in Narberth, PA. You will provide patient visits, perform assessments, develop care plans, and educate clients and families in rehabilitative care.

The ideal candidate holds a PA PT license with at least 1 year of experience, is comfortable using EMR, and can manage time effectively in a home health setting. A $10,000 sign-on bonus is offered for full-time staff.

Qualifications

• Current PA Physical Therapist license.
• Minimum of one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, strength and endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair with evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan and coordinate with nursing in the multidisciplinary plan.
• Educate and instruct clients, families, or other representatives in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Responsibilities for a PT - Physical Therapist include:
