BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist to perform visits in Voorhees, NJ, with a per-visit compensation model. The role covers home health and outpatient visits for adult and geriatric clients in Camden and Burlington Counties.

You will work full-time in a staff capacity within a senior living office, with emphasis on quality and patient-focused care. Responsibilities include conducting mobility and diagnostic assessments, creating and updating individualized

Qualifications

• Must hold an active New Jersey Physical Therapist license.
• Graduation from an accredited PT program as required by APTA/ABHAEAC.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; problem solving is essential.
• Solid computer skills; EMR experience preferred.

Responsabilités

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, balance, strength, safety.
• Implement wound care and protective interventions.
• Develop and adjust individualized care plans with manual therapy, gait training, balance.
• Collaborate with nursing to revise multidisciplinary care plans as needed.
• Educate clients and families on rehabilitative care for safety and independence.
• Accurately document observations and interventions using EMR tablets.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a full time opportunity performing Home Health and Outpatient visits for oursenior living office, located in Moorestown, NJ. This office services adult and geriatric clients on a per visit basis in territories throughout Camden County and Burlington County, NJ.

This position is doing both Home Health (Med A) and Outpatient (Med B) visits with adult and geriatric clients.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications

• A current NJPhysical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information

Base Pay, depending upon qualifications and subject to negotiation:
$63.00-$70.00 per point
