BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist in Mickleton, NJ to perform Start of Care visits and provide skilled home health PT across Gloucester County.

You will assess patients, create and adjust treatment plans, and educate families to promote independence and safety in the home. Applicants should hold an NJ PT license with at least one year of PT experience and be proficient with EMR documentation.

• Active New Jersey Physical Therapist license (PT).
• Minimum 1 year of recent PT experience.
• Graduation from APTA/AMA-approved program; transcript or diploma evidence.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Prior EMR experience is preferred.

Responsabilidades

• Conduct Start of Care (SOC) visits at clients' homes in designated territories.
• Perform diagnostic tests and measurements (range of motion, gait, balance, strength).
• Implement wound care and protective interventions.
• Develop and adjust individualized PT care plans with therapy interventions.
• Collaborate with nursing and multidisciplinary team on care plans.
• Educate clients and families on rehabilitative care for home safety and independence.
• Document observations and interventions using the tablet-based EMR.

BAYADA at Inspira Home Health is currently seeking an experienced AdmissionsPhysical Therapist, for a full-time opportunity performing home health visits in territories throughout Gloucester County, NJ.

Admissions PT's will complete OASIS Starts of Care (SOCs) for adult and geriatric patients. Home Health and OASIS experience is required for this position.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities:

• Make home visits to clients in designated geographic territories and conduct Start of Care (Admissions) visits.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications:

• A current NJPhysical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:
$60.00 - $68.00 per point
