BAYADA Home Health Care is hiring a Home Health Physical Therapist to provide in-home visits for adult and geriatric clients in Maplewood, NJ. A current NJ PT license and at least one year of experience are required.

You will join a multidisciplinary team delivering rehabilitative care to help clients stay safely at home. Responsibilities include performing assessments, creating and adjusting care plans, educating clients, and documenting care in a state-of-the-art EMR system.

Qualifications

• A current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent work experience as a Physical Therapist/PT.
• Graduate from a program approved by the APTA or CMAA/AAHEA accreditation (as indicated by transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilités

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, balance, strength, etc.).
• Implement wound care interventions and promote wound repair.
• Develop and implement individualized care plans (therex, gait training, balance).
• Assess and revise PT care plan in collaboration with nursing and team.
• Educate clients and families on rehabilitative care and self-management.
• Document observations and evaluations in the EMR.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for an opportunity performing home health visits for adult and geriatric clients in territories throughout the Essex County area. Full Time opportunity available!

One year prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• A current New Jersey Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information: Base Pay, depending upon qualifications and subject to negotiation: $68.00 - $75.00 per point
