Job summary

BAYADA Home Health Care in Elizabeth, New Jersey, is seeking a Home Health Physical Therapist for ongoing home visits with adult and senior clients in Union County. One year of PT experience and NJ PT licensure are required. Sign-on bonus of $20,000 is offered with competitive per-visit pay.

You will assess patients at home, develop and adjust care plans, and collaborate with a multi-disciplinary team to support safe, independent living.

Qualifications

• A current New Jersey Physical Therapist license.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Implement wound care interventions with evidence-based dressings.
• Develop and implement individualized care plans including therapeutic exercises and gait training.
• Continuously assess and revise the care plan with nursing input.
• Educate clients and families on rehabilitative care to promote independence.
• Document observations and interventions in an EMR on a tablet.

BAYADA is currently seeking an experienced Physical Therapist, PT, for an opportunity performing home health visits with our adult and senior clients located throughout the Union County, NJ area.

One year prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist:

• A current New JerseyPhysical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$65.00 - $75.00 per point
