BAYADA Home Health Care, Inc. in Duenweg, Missouri seeks a compassionate Home Health Physical Therapist to deliver 1:1 PT in patients' homes across Joplin and surrounding areas. Full-time and part-time roles available, with the option of per diem schedules.

You will perform home visits, assess mobility and functional status, and help patients recover after injury, illness, surgery, or hospitalization while enjoying supportive training and a collaborative care team.

Qualifications

• Active and current PT license.
• One year of clinical experience preferred, new grads welcome to apply!
• Previous home health care experience welcomed, but not required. We will train you!
• Ability to travel locally to cases as assigned

Responsibilities

• Visit clients in their homes to assist recovery after injury, illness, surgery, or hospitalization.
• Perform mobility, range of motion, strength, balance, and gait assessments.
• Develop and implement individualized PT care plans.
• Continually assess and revise the PT care plan and participate in multidisciplinary care.
• Educate clients and families on rehabilitative care and safety.
• Accurately document observations and interventions on a tablet.

We are recruiting for a compassionate Home Health Physical Therapist (PT) to reliably serve patients in Joplin and surrounding areas. We full-time and part-time positions available.

What you'll do:

Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
