Adventist Health in Tillamook, Oregon is seeking a Home Health Physical Therapist to join our patient-centered team. The role focuses on restoring function and improving mobility for home-based patients in the community.

You will conduct evaluations, develop individualized treatment plans, document outcomes, supervise assistants, and collaborate with an interdisciplinary team. A DPT is required with licensure in applicable states, CPR/BLS, and a valid driver's license to support field-based care.

Qualifications

• Education: Doctor of Physical Therapy (DPT) degree required; only if license obtained on or after January 2016; or foreign equivalent.
• Licenses/Certifications: Physical Therapy licensure in all applicable states of practice required.
• CPR or BLS certification from an approved vendor per policy required.
• Valid Driver's License and must be at least 21 years old.

• Completes evaluations to ensure quality standards and addresses assessment areas that require attention, using appropriate clinical techniques and tools, and establishes individualized treatment plans for patients.
• Provides therapy interventions using appropriate treatment protocols and techniques, establishing individualized care and discharge plans and ensuring plans are communicated to appropriate parties.
• Completes comprehensive and accurate clinical documentation within established deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.

Job Summary:

Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.

Job Requirements:
