Job summary

Elara Caring is seeking a Home Health Physical Therapist to provide direct patient care in patients' homes in Pittsford, Michigan. You will evaluate functional needs, design and implement personalized treatment plans, and collaborate with a multidisciplinary team to achieve optimal outcomes.

You will travel up to 50% within the service area, maintain detailed documentation, and contribute to the community-based care mission. A $10K sign-on bonus is offered.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program.
• Current, unrestricted PT license in Michigan.
• At least 1 year of experience in healthcare.
• Willingness to travel up to 50% within your coverage area.
• Reliable transportation and valid driver's license.

What You’ll Bring

• Reliable transportation, valid driver's license, and current auto insurance
• Ability to travel up to 50% within your coverage area
