Job summary

Park Nicollet Health Services is seeking a Physical Therapist to join our Home Care/Hospice team in Minnesota. The role focuses on direct patient care, outcomes assessments, and care planning for patients in their homes. You will collaborate with families and clinicians to improve care and satisfaction.

The position offers a 4-day work week, Monday to Friday, 8:00 am - 4:30 pm, with occasional weekends. Licensure and BLS certification are required; prior homecare experience is preferred.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy.
• Current Minnesota Physical Therapy license.
• Current BLS certification required and maintained.
• Knowledge of age-related growth and development, and OASIS data set.

Responsibilities

• Provide direct patient care and participate in process improvements.
• Act as case manager to assess and evaluate patient care needs.
• Coordinate medical, social, and financial resources to develop a plan of care.
• Communicate professionally with staff, patients, families, physicians and others.
