BAYADA Home Health Care is seeking a Full-Time Physical Therapist to provide exceptional care to adult and geriatric patients in their homes across Quincy, Braintree, Holbrook, Randolph and surrounding towns. Prior home health experience is not required and new grads are encouraged to apply.

A $10,000 sign-on bonus is offered. Responsibilities include visiting clients at home, performing mobility and strength assessments, and developing individualized care plans with collaborative team input.

Qualifikationen

• Active Massachusetts PT license required.
• Home health experience not required; new grads encouraged to apply.

• Make visits to clients in their homes to assess and treat.
• Perform diagnostic tests and measurements for mobility, gait, strength and balance.
• Develop and implement individualized care plans with interventions.
• Collaborate with nursing and multidisciplinary teams to adjust care plans.
• Educate clients and families on rehabilitative activities to promote safety and independence.
• Document observations and interventions accurately in the electronic record.
